Full Job Description
- Physical Therapist evaluates and treats patients recovering from injury, disease or surgery, etc in a home care environment.
- Establishes individualized treatment plans for Home Health/Hospice patients.
- Maintains records by tracking goals and progress for each patient and adjusting or adapting plans as needed.
- Individualized treatments may include a variety of methods such as, exercises, hands-on therapy, stretching, and/or equipment.
- Physical Therapist evaluates patients and establishes written plans of treatment with short- and long-term goals.
- PT keeps patients and their families informed regarding in-home treatment options and the end goal or projected outcome of treatment.
- Good communication skills needed as they work with and interact with team members, patients, providers, and other medical professionals.
- Travel to patient homes and outside facilities required
- Must have current Montana PT license
- BLS Certified
- Valid, Clean Driver's License required
