Home Health Case Manager

Job Responsibilities
- Develop individualized care plans for patients based on their specific health needs and goals.
- Coordinate and oversee the delivery of healthcare services to patients in their homes.
- Conduct regular assessments to evaluate patient progress and adjust care plans as necessary.
- Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure comprehensive patient care.
- Serve as the primary point of contact for patients and their families, providing education and support as needed.
- Monitor and document patient care activities, ensuring compliance with healthcare regulations and standards.
- Identify and address any barriers to care, facilitating access to necessary resources and services.
- Manage the transition of patients from acute care settings to home health care.
- Ensure timely and accurate submission of all required documentation and reports.
- Participate in case conferences and team meetings to discuss patient progress and care strategies.
- Advocate for the needs and preferences of patients in the development and implementation of their care plans.
- Maintain up-to-date knowledge of home health care best practices and regulatory requirements.
Job Requirements
- Minimum of 2 years of clinical nursing experience, preferably in home health or community health setting.
- Proficiency in Spanish preferred.
- Valid Registered Nurse (RN) license in the state of employment.
- Bachelor's degree in Nursing preferred.
- Proven experience in case management or care coordination.
- Strong understanding of home health care services and regulations.
- Excellent organizational and time-management skills.
- Ability to work independently and manage multiple priorities.
- Proficient in electronic health records (EHR) and case management software.
- Strong interpersonal and communication skills.
- Ability to collaborate effectively with patients, families, and interdisciplinary teams.
- Knowledge of healthcare reimbursement and insurance rules.
- Valid driver's license and reliable transportation.
- Current CPR certification.
- Ability to handle sensitive and confidential information with discretion.
- Flexibility to work varied hours to meet patient needs.
- Can be Per Diem or Salary
